I'm not sure I get this graph... it sounds like the more people who "take" credit, the greater the likelihood of success.

I think the whole point is that no matter how many people work on a thing, there are a lot more people willing to take credit than blame.

I would alter the "team effort" to "Management Win" and "Individual failure" to "Non-systenic Failure"
That way its the usual management win win arrangement

This could have been a four quadrant graph, actually, with fault being the negative of success.
